Consent for Telehealth Consultation

Doctors on Demand is a Virtual Platform. Our doctors are General Practitioners registered with the Health Professions Council of South Africa.

All patient information is encrypted and stored securely. We will never share your phone number or any personal data with anyone outside of ‘Doctors on Demand’.

I consent that my/my child’s information may be transferred to another healthcare practitioner or other appropriate third party when this has been discussed with me and I have consented to such, in the consultation.

I understand that there are limitations when choosing a telehealth consultation. Therefore, I agree that the doctor will decide whether the condition being diagnosed or treated is appropriate for a Telehealth consultation, and if there is a disagreement regarding this I will ensure that I present myself/my child to an alternate service for an in person consultation.

I agree that if I am in need of a  chronic renewal of a script I will need to provide past scripts/ letters from your previous doctor/ lab results/ vital signs readings where appropriate.
